Causes: Hospices, Human Services
Mission: To optimize health and independence and to keep residents of our community healthy and living independently wherever they call home, to provide expert, compassionate and innovative care in collaboration with individuals, other members of the health care team and community organizations.
Programs: Certified hospice benefit - activities primarily funded by medicare. Services such as nursing, home health aide, social work, spiritual and nutrition are offered to terminally ill patients requiring treatment and medical care. A total of 799 patients were provided at least one level of hospice care with 21,876 total hospice "days" of service provided for the twelve months ending 6/30/17.
inpatient hospice - a specific level of hospice care provides 24 hour palliative nursing. Physician/crnp, personal care, social work, nutrition and spiritual are offered to terminally ill patients requiring treatment and medical care in an inpatient facility setting. A total of 369 patients were provided general inpatient care for the twelve months ending 6/30/17.
palliative care - specialized care funded by medicare as well as various other insurances. Services are offered in the following disciplines: registered nurses, licensed practical nurses, physical therapists and physical therapy assitants, occupational therapy and certified occupational therapy assistants, speech therapy, medical social workers, home health aides, and nutritionists. The goal of such services is to provide patients diagnosed with an advanced illness an enhanced quality of life while undergoing their treatment plan of care. A total of 738 patients were provided services for the twelve months ending 6/30/17.
