Homefirst focuses on helping its members remain in the comfortable surroundings of their own homes and ensures accessible, appropriate, high quality and cost-effective delivery by coordinating acute and long-term care services. These services include: nurse supervision and home visits, coordination of medicaid and medicare services, and comprehensive assessment of environmental, social and caregiver factors, as well as respite services to alleviate caregiver stress. Total membership increased from 3,461 in 2009 to 3,543 in 2010.
