Mission: To transform the health and lives of those in need through the love
Programs: Hchc provided behaviorally-enhanced primary care services for both insured and uninsured patients living in various zip codes within douglas, jefferson, osage, franklin and leavenworth counties in fiscal 2017. This care was provided during approximately 12,000 patient encounters. Through the year hchc had 3 physicians, 3 nurse practioners, 1 dentist, 1 behavioral health consultant, 1 licensed clinical social worker, and 1 physhical therapist.
