Mission: The healthy greater newark accountable care organization is a diverse team of newark provider partners that includes hospital, federally qualified health centers, behavioral health, substance use disorder and community representation. Aco partners are working together to improve health and maximize efficiencies through provider level practice transformation and community engagement using data driven strategies.
Programs: New jersey medicaid aco pilot supports the design and implementation of a medicaid accountable care pilot in three nj communities (newark, trenton and camden) to explore innovative system re-design including (a) testing the aco as an alternative to managed care, (b) evaluating how care management and care coordination could be delivered to high risk, high cost utilizers, (c) stretching the role of medicaid beyond medical services to integrate social services and, (d) testing payment reform models including pay for performance metrics and incentives. Aco infrastructure (the nicholson foundation) supports the medicaid aco pilot by funding core aco staff, specifically an executive director and data analyst, and related efforts to research, design and implement aco financing and care management models.
