Mission: Health outreach partners mission is to build strong, effective, and sustainable grassroots health models by partnering with local community-based organizations across the country in order to improve the quality of life of low-income, vulnerable, and underserved populations.
Programs: Information services & resources: during the 2016-2017 fiscal year, hop provided 15 webinars and 18 training workshops that reached 1,749 representatives of community health centers and primary care associations: 1,358 and 391 representatives, respectively. In addition, hop released five new resources. Three of these publications addressed the topic of transportation and access to care: an infobook titled, where are we now and where can we go? ; the rides to wellness community scan project report; and the transportation and health access: a quality improvement toolkit resource. Hop also developed a self-care resource, taking care of ourselves so we can take care of others, for outreach program staff at community health centers. Furthermore, hop issued the, outreach and value-based care: impacting health care delivery and cost through integrated community health outreach programs, resource, a complement to the hop outreach business value (obv) toolkit.
training: during the 2016-2017 fiscal year, hop delivered 15 regional trainings to a total of 181 community health center and primary care association representatives.
program consultation: hop provided 12 client-centered consultations during the 2016-2017 fiscal year, including one (1) intensive consultation and eleven (11) targeted consultations to community health centers and primary care associations.
