In addition to our skilled health care practitioners, dedicated professionals in the areas of social services and rehabilitative recreation see to the broader emotional and social needs of our patients. Our voluntary board of directors is comprised of local professional and civic minded individuals, who, without compensation, donate their time to fulfilling the home's mission.
Programs: The organization rendered 43,341 patient days of skilled medical & nursing and rehabilitative care to aged men & women. Fieldhome-holy comforter is a health care and community services organization, established in 1879. Our beautiful 100+ acre campus is located on the border of cortlandt manor and yorktown, in upper-westchester county, ny. In 2014 we served approximately 700 patients primarily from westchester, putnam, dutchess and rockland counties. We work with our local hospitals as well as many hospitals in new york city. We are an equal opportunity employer with over 253 rns, lpns, cnas, dietary aides, housekeepers, maintenance engineers and clerical workers. Fieldhome- holy comforter provides the most comprehensive post-surgical rehabilitation program outside the sub-acute hospital setting. Our 1,500 sq ft rehabilitation treatment center, specializes in four areas: post-surgical, orthopedic, neurological and cardio-pulmonary rehabilitation. We offer a 7 day a week rehabilitation program with our a+ rated and credentialed therapy staff. The rehabilitation treatment center offers a wide range of treatment modalities, including ultrasound, electric stimulation, pain management therapy and heat & ice therapy, with a high tech therapeutic cooling system originally designed for use by professional and college athletes. Patients receive a full complement of comprehensive services, including physical and speech therapies as well as occupational therapy in a separate occupational therapy suite. The o. T. Suite is complete with an actual bedroom, bathroom and kitchen so patients gain the confidence needed to safely return home. In 2014 we rendered 25,160 therapy treatments to our patients. A 50-bed rehabilitation unit is integrated with the center and offers many private rooms. Admissions are accepted 7 days a week and most insurances are accepted. Our traditional long-term skilled nursing unit, is mission driven and committed to the ideals of person centered care and to the value of individualized attention and consistent staff assignments. Our long established reputation of nursing excellence is one that is built on our 130 years of experience. Our residents also benefit from inter-generational programs with the children of the early learning center at fieldhome. We have a high nurse to resident ratio and take great pride in our staff, who are professionally trained to manage special needs such as wound care, infusion therapy, pain management and palliative/comfort care. Our skilled nursing program provides a comprehensive list of support services, including: full rehabilitative therapy, nutritional assessment and support, laboratory and diagnostic services, dental care, therapeutic care, social work services, psychological services, vision care and religious services. Respite and hospice/comfort care are also available. In 2012 field home holy comforter was the recipient of a heal grant award in the amount of $8,000,000. This award was from new york state heal ny phase 21, restructuring initiatives in medicaid redesign. We were required to extinguish our long term debt, which was done in 2012. The grant had several initiatives which included the downsizing of the long term care beds from 150 to 75. This was accomplished in november 2013, install a new emr/fiscal system, and create a social adult day care which was accomplished in early 2014. These initiatives will allow us to realign our campus and implement a much needed community based program.
