These health care services are provided to meet the health care needs of the elderly and others within the community. Home care services involve visits to the patient's home and include skilled nursing, physical therapy, occupational therapy, speech therapy, infusion therapy, medical social worker visits, home health aide visits and in-home emergency response monitoring system. The case manager works with doctors, other team members, and caregivers to develop a plan of care.
