Mission: Esperanza Health Center opened its doors in 1989 as a small clinic in the heart of North Philadelphia's Latino community with a few paid and volunteer staff. Today, Esperanza provides whole-person health care services from three sites in North Philadelphia - Fifth Street, Hunting Park, and Kensington - serving over 14,000 patients annually. Esperanza's model of care seeks to address patients' physical, emotional, & spiritual needs. No one is denied care because of inability to pay; payment for individuals without insurance is on a sliding scale based on income. Services include: adult and pediatric primary care; behavioral helath care; dental care; preventive care and immunizations; OB/GYN care; spiritual care and counseling; nutrition counseling; comprehensive HIV/AIDS medical treatment and counseling; and, community health and wellness programs.
Results: Esperanza Health Center is designated as a patient-centered medical home, and was recognized as a Health Center Quality Leader in 2017 and 2018 by the Health Resources and Services Administration (HRSA) for ranking in the top 30 percent of all community health centers nation-wide in measures of clinical quality of care. Esperanza's staff of 210 employees includes 30 primary care clinicians (physicians, nurse practitioners and dentists).
Target demographics: indivdiuals and families to obtain high-quality medical, dental and health and wellness services
Direct beneficiaries per year: In 2018, we served over 14,000 patients of all ages, and served many other community residents through our community health and wellness programs.
Geographic areas served: the low-income, predominantly Latino and African-American communities of North Philadelphia
Programs: primary adult and pediatric medical care; dental care; preventive care and immunizations; OB/GYN care; spiritual care and counseling; nutrition counseling; comprehensive HIV/AIDS medical treatment and counseling; and, community health and wellness programs. All of these services and programs are provided from a Christian perspective, and are offered without regard for ability to pay.
