Mission: To be a catalyst providing individuals with mental health diagnoses opportunities for choice and development of personal goals, using the four principles of self determination: freedom, authority, responsibility, and support.
Programs: The peer mentor program provides on-site peer mentor services to the tenants of two local apartment complexes in clackamas county. Our peer mentors provide problem solving and crisis support, coordinate group activities, facilitate community integration and work activities, and offer skills training to tenants. We support tenants to maintain their supported community housing and continue on their path to mental health recovery.
based upon the program participants desire to move, the sustainable housing brokerage program serves individuals stepping down from institutional settings, group homes, as well as transition aged youth and provides the necessary supports and resources to transition the participants to a setting of greater independence. Participants receive a rental subsidy and work with a resource broker to create a person-directed plan which helps them set goals for themselves in many areas of their life. The brokerage program offers participants the freedom and the resources to investigate, determine and secure the products and services they themselves consider most effective for their mental health and well being.
