Causes: Centers to Support the Independence of Specific Populations, Civil Rights, Disabilities, Disabled Persons Rights, Human Services
Mission: Connecting people with disabilities to resources and opportunities, while developing communities where everyone can participate, contribute & belong.
Programs: To support the determination of individuals with disabilities to lead dignified, independent lives and to heighten community awareness of physical & social barriers which restrict independence. Fiscal year 2017 significant outcomes include, but are not limited to: community outcomes: 19 private, public, government and nonprofit entities increased their physical or programmatic accessibility 153 hours of community and systems advocacy led to increased availability and access to supports and services for community based living. 798 hours of adaaa titles ii and iii technical assistance led to increased physical and programmatic accessibility. 1,456 hours of community education and public information led to increased community awareness, access and availability of supports to community-based independent living. 64 area employers and organizations were educated about the value and employability of people with disabilities in the workplace. 16 area nursing homes and 89 organizations and service providers were educated about nft program. Individual outcomes: 39 consumers acquired or increased self-advocacy skills. 1013 individuals received information & referral services and acquired increased access to desired knowledge, goods, services and the supports necessary for enhanced independence and community-based living. Through peer support & mentoring, 82 consumers developed or maintained natural and other community-based supports. 57 individuals acquired desired independent living skills and outcomes for independent living. Through individual advocacy efforts, 38 individuals acquired access to benefits, services and programs to which they were entitled. Education 343 michigan rehabilitation services customers about disability network/lakeshore and other community-based services. 60 individuals received necessary assistive technology and/or durable medical equipment through the assistive technology loanfund or disablity network/lakeshore loan closet. Through community transitions services, 12 nursing facility residents transitioned from a nursing facility to community-based living options of their choice; 15 persons received follow-along support services and maintainted community-based living; 18 persons at-risk for institutionalization attained diversion-related outcomes; 16 post-high school youth assisted with transition to post-secondary educations vocational training, independent living and/or community participation. As referred from michigan rehabilitation services, 103 eligible youth from 8 school districts received pre-employment transition services and were educated about the world and reality of work. Through employment-related services, 49 persons attained and/or maintained competitive part and full-time employment.
