Mission: To partner with individuals, families, providers, and the community to promote growth and resilience in individuals living in central iowa with disabilities such as mental illness, brain injury, intellectual disabilities and other developmental disabilities.
Programs: Case management services are available to individuals with a brain injury, intellectual disability or other developmental disabilities and are designed for individuals who demonstrate a need for coordination and assistance in at least two areas of life. Case management is an individually centered, family and community focused process that assists individuals in identifying, securing and sustaining the resources necessary to live, work and socialize within their community.
integrated services utilizes a team approach to provide "wrap-around" support services to adults with disabilities. It is designed for individuals with multiple areas of support needs who have a mental illness, intellectual disability or other developmental disability. Teams assist individuals in accessing appropriate and safe housing, needed medical and behavioral health services and interrelated social, vocational and educational services.
the integrated health home program provides whole health coordinated care for youth and adults, age 16 and older, with a serious mental illness or serious emotional disturbance who are medicaid eligible. This program is designed to address the unique mental health, physical and social needs of young people and their families as they transition into adulthood.
service coordination services provide support to individuals who may not be eligible for medicaid who have an intellectual disability, other developmental disability or mental illness currently under treatment. Service coordination is often where an individual in crisis is supported in navigating the disability support system. Service coordination services assist individuals in qualifying for and accessing the needed supports to live, work and socialize within their community. ; momentum is an art access program that is a series of workshops and studio space for providing opportunities for members to engage, learn and create art as a way to promote growth and resilience in the individuals and families csa serves.
