Mission: The mission of The Coordinating Center is to partner with families, children and adults with special health care needs and disabilities and those who support them in the community to achieve their aspirations for independence, health, inclusion, dilgnity and quality of life.
Programs: The center provides case management services for medicaid recipients with complex medical needs, enabling them to live in the community of their choice with medical services they receive. This is the "rare and expensive case management" or "rem" program, serving 3,981 clients.
adult community services (acs, formerly the living at home waiver) - under a contract with the maryland state department of human resources, community services administration, the organization provides home care management services to 2,303 adults with physical disabilities.
home and community case management services are provided to individuals with various complex medical needs. This program, "model waiver", serves 200 clients.
