Mission: The community health centers of burlington (chcb) is a nonprofit health and human services orgainzaation whose goegraphic reach spans vermont's most populated and urban county, chittenden county, and the rural southern portion of grand isle county, representing a total of 150,000 vermonters. Through our 24-year successful history as our region's sole federally qualified health center, chcb has served our community as a health care home for vermonters of all walks of life, regardless of ability to pay.
Programs: The community health centers of burlington (chcb) is a primary care family practice and a federally qualified health center offering medical, dental, laboratory, psychiatry, mental health, substance abuse and case management services as part of an integrated medical home. Chcb offers a sliding fee financial assistance program for all services based on income within our defined service area, and to those qualified as homeless at no cost to the patient. Chcb serves all members of the community as patients regardless of life circumstances, income or insurance status. Chcb's five health centers are located in burlington and south hero, vt. The following are a few of the fy14 achievements that chcb has accomplished. (continued on schedule o) -chcb's shelter plus care program has ranked the first or second top performing coc hud program in the nation for the last serval years. Furthermore, chcb's housing case worker program has served 112 households in less than two year and has housed 35 households, comprised of 78 people. In all, this program has housed 77 people with a 91% success rate. - chcb's dental program provides access to dental care for low-income, underserved vt adults and children. Chcb's school-based dental center is designed to lift barriers to dental care for low-income children who arrive at school without a dental home. Our program ensures kids within the burlington school district can receive dental checkups, hygiene restorations and fillings. Additionally, all school children receive toothbrushes, dental floss, and education concerning best dental hygiene practices. This year, over 700 children received care. Our adult oral health program also served a record number of patients, which included over 7,000 preventive visits. - chcb houses vt's only at-home physician visiting programs. With 17% of all of our patient visits interpreter-assisted, chcb took the initiative this year to add an additional block of appointments strictly for incoming refugees. - finally, chcb also expanded services for our patients with mental illness. Our behavioral health program now includes a psychiatrist who treats all ages, three psychiatric nurse practitioners and a psychiatric walk-in clinic.
