Causes: Group Health Practices, Health
Mission: C3's mission is to leverage the collective strengths of federally qualified health centers (fqhc) to improve hte health and wellness of the people we serve. We are a new, 501(c)(3) not-for-profit, accountable care organization (aco) govenred by fqhcs.
Programs: C3 managed an accountable care organization pilot program from december, 2016 through november 30, 2017. The aco is made up entirely of 15 federally qualified health centers.
