Mission: The mission of city impact is to share the message of hope and reconciliation through the gospel of jesus christ, to youth, families and the other lives we touch; earning the right to meet their spiritual, educational, physical and emotional needs through outreach programs focused on all ethnic groups.
Programs: Transformation works, a collaboration with local law enforcement agencies that provided case management services to gang involved youth and youth at high-risk for gang involvement (ages 12-24). Participants received one-to-one interventions and worked with their case manager to set individual goals, maximize personal strengths and address barriers to success. In addition, family members were provided support, resources and referrals for services. 219 youth were provided intensive case management services to help deter them away from high-risk and gang activity. 131 families (511 individuals) were provided with support and resources for fy17. Student and family empowerment (safe) provides a safety net of one-on-one counseling and case management for special education students in elementary, middle and high school who face emotional and social challenges identified in their individualized educational plan (iep) prepared by schools. This school year both attendance rates and grade point averages improved. Individual goals were reviewed and evaluated every quarter (9 weeks). 192 special education students were assisted weekly with the social/emotional goals stated in their individualized education plan for fy17. City impact's fillmore middle shcool (ms) and high school (hs) couseling services is a comprehensive program that safely and effectively provides therapeutic services to targeted youth, referred by school personnel. The program combined prevention and intervention services through evidence-based programs, practices, and strategies. 79 students were seen through the fillmore ms and hs program for fy17.
