Mission: The mission of child to family connections is to partner with individuals and families to recognize their strengths, build relationships, and provide the support and encouragement they need to succeed.

Services provided at child to family connections include: foster care, adoptive home studies, home & community habilitation services, family group decision making, family living, learning to live, an independent living program for teens, and strengthening families program. Foster carechild to family connections, inc. Places foster children in qualified foster homes and provides training and education to the foster family. There were approximately 17 foster children served during the fiscal year. Home & community habilitation serviceshabilitation services are provided to individuals with special needs to help build the confidence and skills needed to be successful in life. Our support specialists guide individuals in learning, maintaining, and improving the skills and confidence necessary to live independently and to become positive, contributing members of the community. Services are provided at home and in the community. Family livingfamily living homes gives individuals with unique needs the opportunity to live in a caring, supportive household. Individuals supported in family living homes include children and adults with a wide variety of needs and challenges. These homes enrich the lives of individuals by offering another residential living option. Independent livingthe mission of learning 2 live is to educate at-risk youth in our community through a strong, yet entertaining learning atmosphere, presenting them with the resources necessary to live independently, to provide support for them by collaborating with the community to create a support system of peers and adults; and to instill in each youth a desire to succeed, based on their own personal strengths. Strengthening families program the strengthening families program for parents and youth 10-14 is a widespread, evidence-based prevention program which enhances parents' general child management skills, parent-child affective relationships, and family communication. Based on a developmental model, sfp assumes that increasing the family's protective processes while decreasing its potential risk factors can alter a child's future, so that problem behaviors can be reduced or avoided. In addition, sfp seeks to delay the onset of adolescent alcohol and substance abuse. Family group decision making. Fgdm is a process that empowers families by placing them in charge of developing their own plan which will guide their future. Once a plan is agreed upon by everyone involved, it will become the family's primary plan and will be monitored by the referral source. Transitional living program. The transitional living program provides housing for young adults ages 18-24 that are at-risk of becoming homeless or are already homeless. The program provides support and services to address substance abuse and trauma issues; address, support, and encourage employment and educational skills, and to provide skills that increase their ability to live independently.
