Mission: The primary and specific purpose for which this corporation is formed is to promote and support the social welfare of individuals and families that have been impacted by multiple sclerosis neurological disorders resulting in long-term physical and/or mental diabilities and to improve their quality of life. This purpose will be accomplished through medical and social service training, education, and the provision of services that will allow individuals to achieve the maximum level of independence and autonomy possible and to be treated with the meximum level of respect and dignity.
Programs: Individual case management services offered to assist clients in a variety of systems such as: navigating the disability process, education about multiple sclerosis and parkinson's disease, obtaining medical equipment, addressing accessibility issues and finding resources for home modifications, facilitating referrals to various healthcare and rehabilitation specialists, and assistance with enrolling in various state and federal programs. Ccqlp accomplished home visits serving individuals in the tri-county area; assisted clients and their families in identifying and accomplishing care plan goals that are classified under eight areas of need: health and healthcare, finances, social support, functional abilities, activities of daily living, community services, home environment and perceived future needs; and made over 4,594 moments of contact with clients and for clients. 429 clients received case management services.
offered weekly water aerobics classes to persons with multiple sclerosis and their caregivers. The in-water trainer directs clients in an aerobic program for stimulating ciculation through arms and legs, working on stretching and balance, and providing a comfortable environment for exercise and camaraderie. Water aerobics classes offered to individuals with ms or pd and their caregivers. 25 clients received water aerobics services.
offered monthly support group meetings to persons with multiple sclerosis and their family and care providers. Many of the support group meetings are supplemented with a special topic or theme, with many topics accompanied by a special guest speaker - hosted support groups for individuals with ms and their family members. 48 clients received support group services.
provided educational monthly e-newsletters, seminars and a website to inform health care organizations, non-health care groups, and the consortium of multiple sclerosis to advocate and raise awareness of the realities of living with a chronic illness. Provided individual advocacy for their clients related to navigating the healthcare, welfare, and legal systems - hosted collaborative workshops on advance directives, fall rpevention and home modifications; co-promoted educational events sponsored by pharmaceutical companies regarding the treatment and relapse management therapies; and participated in outreach events in order to increase their engagement with the public and other non-profits and to bring awareness about the services they provide. 109 clients received monthly newsletters for a total of 1,308 newsletters.
equipment reuse program was developed to provide essential mobility equipment (for loan) to individuals who need a mobility device to get around while they wait for their prescribed device - 42 mobility devices or adaptive equipment donated to clients in need.
The MS Quality of Life Project was my lifeline for me and my late husband. I turned to them for everyday practical needs as well for support with coping with major health decisions. I remember the day when my husband began to have trouble walking and we were stuck on a sidewalk in Pacific Grove. In a panic, I called the MS Quality of Life Project. They dropped what they were doing and sent an angel with a walker to the sidewalk where we were stuck. We were able to complete our very important doctor’s visit. Another instance I called from CHOMP hospital and again I received helpful information in a time of crisis. I was able to make important decisions based on their help. My daughter called MS Quality of Life to interview one of their staff for her senior project in high school. She was able to present a well informed talk about MS educating the community. I could never say enough wonderful things about MS Quality of Life. They are outstanding and excellent in every way. They have been an amazing gift to me. Thank you MS Quality of Life from the bottom of my heart!
I really do not know what I would have done with out Msqlp the past 2 years.
were it not for them I would not have known about many resources available, especially when my condition so quickly progressed to the point where I could no longer walk and needed a loaner wheelchair fast. They are always so helpful when I call, even when I am frustrated with processes like trying to get a chair of my own and issues with my doctor and medicare. I tell people about them all the time. I am so grateful!

As a practicing R.N. for over 35 years and working in many challenging fields of practice, I am so very glad to read of the importance of gratitude in your newsletter. This behavior is clearly a cellular changer. Your newsletter is always a joy to read. Getting to know the staff and volunteers on a level that makes me feel close to them even though I have only read about there passion for the work they do in the newsletter. I firmly believe the core of life is sharing and being available to each other, and your group delivers on each account. You all are providing more than you will ever know to those who may not be able to express the gratitude they feel in their hearts. So I am here to say Thank You for your love and service!~!~!
