Causes: Health
Mission: The caresource foundation is organized and shall be operated solely to improve overall community health, address health-related issues and foster a positive image for the caresource family of companies in the counties it serves. The caresource foundation improves the lives of the underserved by providing grants to non-profit organizations who can significantly impact individuals and families through a broad array of health and human services and programs. These investments target the neediest populations in ohio, and provide innovative solutions to critical health care needs and create meaningful collaboration. The caresource foundation's mission is to advance the health and well-being of underserved people in the communities we serve by providing innovative solutions and funding to address health care needs.
Programs: The caresource foundation provides opportunities to advance innovative solutions and address health care needs of the underserved through grants, outreach, medical expertise, strategic community partnerships and volunteerism. The foundation funds responsive grants for targeted areas of need, which are given to individual non-profit organizations or a collaboration of non-profits. Funding is in the following priority areas: (1) children's health, including critical health trends, children in poverty, and access to care, (2) crisis support for women and children, including domestic violence, child abuse, and homelessness, and (3) special needs populations, including the elderly, and those with disabilities. The caresource foundation also funds signature grants with numerous regional, state and/or national partners all focused on creating sustainable, high-impact change. The caresource foundation funded the following signature grants in 2014: (1) a $240,000 grant to the dayton foundation to help create dayton works (now miami valley works) to empower people in poverty to achieve self-sufficiency through long-term employment and career development. Miami valley works' vision is to eliminate chronic generational poverty in dayton, ohio. Funding for this signature grant will occur over a two-year period beginning in 2014 and ending in 2015; (2) a $220,000 grant was awarded to interfaith hospitality network to provide essential supportive services for the formerly/potentially homeless population being served at mulberry terrace in springfield, ohio. The 34-unit complex is the first of its kind in springfield to offer a range of services on site that are aimed at helping people fight homelessness. Onsite at mulberry terrace is a community health center that provides case management services to every resident, ensuring access to behavioral and physical health services, all aimed at assisting individuals and families sustain their housing. The caresource foundation grant will support on-site case management, a service coordinator and mental health therapy in collaboration with rocking horse clinic. Funding for this signature grant will occur over a two-year period beginning in 2014 and ending in 2015; (3) a $150,000 grant to ohio health foundation was funded to expand the wellness on wheels mobile health clinic in columbus, ohio with a goal of reducing infant mortality in low-income populations. Funding for this signature grant will occur over a two-year period beginning in 2014 and ending in 2015; (4) a $218,000 grant to cincinnati children's hospital medical center was funded to support the check foster care clinic. The check foster care clinic helps coordinate health care for foster youth, particularly those who are aging out of the foster care system, and who are at-risk of critical health and life issues; (5) a $150,000 grant was awarded to the dayton foundation to support the montgomery county ohio college promise program which will help low-income students get to and through college. College promise's goal is to provide children in poverty with the best opportunity for both high school and college graduation while also having an impact on their quality of life and their community. The program provides each college promise student with a supportive mentor that starts at middle school through their senior year in high school. Students who fulfill their pledge will be awarded scholarships to attend college. Funding for this signature grant will occur over a two-year period beginning in 2014 and ending in 2015; (6) a $225,000 grant was awarded to the children's hunger alliance to launch the free breakfast program in up to 14 schools in cleveland, ohio (cuyahoga county). Funding for this signature grant will occur over a two-year period beginning in 2014 and ending in 2015; (7) a $250,000 grant was awarded to march of dimes. This grant will support the march of dimes ohio prematurity institute, a collaboration of world-class scientists, doctors and educators who are working over a 5-year period to address ohio's critical issues around infant mortality. Funding for this signature grant will occur over a two-year period beginning in 2014 and ending in 2015. In addition to these signature grants, $923,815 was provided for responsive grants to a variety of health/human service nonprofits throughout the 88 counties in ohio. This resulted in a total foundation investment of $2,376,815 in 2014 through grant funding. Finally, in support of the caresource foundation's commitment to its charitable purposes, its appointed trustees and committee members are given the ability to recommend that the organization donate certain dollar amounts to their internal revenue code section 501(c)(3) tax exempt charity of choice. Charitable donations in the amount of $24,000 were paid or accrued in 2014 under this policy by the organization.
