Causes: Community Health Systems, Health
Mission: To lead the transformation of primary care in rhode island in the context of an integrated health care system; and to improve the quality of care, the patient experience of care, the affordability of care, and the health of the populations we serve.
Programs: Accomplishments include: average total cost of care for ctc participating practices was 18% lower than comparison group. Ctc has 69 current practices enrolled, 16 graduated practices and 3 new practices for a total of 263 primary care practices participating in our program. Assisted 77 practices to meet the ohic standard for pcmh. Held annual learning collaborative and had 231 participants.
