Mission: Help/psi services corp operates three primary care clinics co-located with adult day health care programs in the bronx, brooklyn, and queens. The model is important for adhc clients that have historically been unsuccessful in establishing and maintaining linages with primary care providers. The program traditionally services clients below the poverty level and specializes in serving those that are infected with hiv and have a history of substance abuse, homelessness, and incarceration. Services will include primary care, internal medicine, gynecology, health education, nutrition, nursing, social work services, family planning, and mental health services.
Programs: Operates four diagnostic and treatment centers designated as federally qualified health centers designated as a homeless provider. The majority of clients will be low-income persons who may have a history of substance abuse, may previously have been homeless, have histories of incarceration, etc.
