Causes: Community Mental Health Centers, Mental Health
Mission: Our mission is to strengthen individuals, families and community life by delivering a behavioral health care and human service system that provides an integrated continuum of essential, responsive, and cost-effective services
Results: Our new (2012) Center for Well-Being has expanded services to our adult population and is poised for growth in integrated care and wellness.
Target demographics: 1 in 4 of us!
Geographic areas served: Clinton, Essex and Franklin counties of NYS
Programs: Clinical services for children and adults, Employee Assistance Program (EAP), Caregiver Respite for elderly as well as families with children receiving services, Domestic Violence Prevention and safe dwellings, Batterers Program based on the Duluth Model, Healthy Families Clinton (HFNY), Children's Case Management, Adult Care Management in a Health Home model, Supported Education, Supported Employment, residential facilities for adults including one for men with substance abuse addiction.
