baptist health ambulatory services, inc. Includes the y healthy living center departments and the jca wellness connexion department, which respectively represent health partnerships with the ymca of florida's first coast and the jewish community alliance (jca). Y healthy living centers two new y healthy living centers were opened in january and february 2017 - 1) in an existing ymca in an at-needs community area of north jacksonville and 2) in an addition to the wellness portion of the medical office building on the baptist health north campus, which is located directly next to the y fitness facility on the first floor. The entire medical campus was built with wellness and prevention in mind. Baptist health continued their commitment to connect with community members where they live, work and recreate through the now five community y healthy living centers in 2017. Both new center locations were chosen due to the community need of health education resources, as the northside and baptist north locations were built in health zones that were rated lowest in overall and specific health outcomes, including diabetes, hypertension, and obesity. Baptist health provided a full-time staff person at each of the y healthy living centers to offer health screenings including blood pressure, body composition, cholesterol, glucose, triglyceride values, care navigation, and health coaching. The northside location included an onsite rn, as the chronic disease prevalence is highest at that particular location. Each center offered a monthly health education schedule that included talk with a doc and ask the expert sessions that were free and open to the community, regardless of membership. Family/parenting classes were also held in the centers for a minimal fee. In fiscal year 2017, there were 770 community member screenings completed (235 in 2016), 3,634 class visits to the hlc (1,925 in 2016), 2,087 unique people served (1030 in 2016), 264 events & classes (121 in 2016), and 1,216 follow-up health coaching sessions (89 in 2016). The centers also added mental health first aid trainings as a priority and held 9 trainings across the 5 centers. The usage of the centers have increased year over year, while also opening the fourth and fifth center. Jca wellness connexion the wellness connexion opened in 2015 and celebrated another year of partnership between baptist health and the jca. Baptist health staffed two onsite full-time nurse wellness coaches at the jca to provide health screenings, care navigation, and health coaching. In fiscal year 2017, the wellness connexion provided 254 free health screenings (269 in 2016) and 779 follow-up health coaching sessions (448 in 2016). (including a mix of community members and employees). There were 306 visits to the baptist health events and classes held at the jca (518 in 2016) and 1,339 total visits to the wellness connexion programming (1,235 in 2016), which included physician and expert-led education.
