Mission: Our mission is to provide a community-based diabetes self-management education program that will serve as an effective intervention for glycemic control. Our targeted demographics are ethnic minorities, underserved populations. The goals of the DSME program are to improve metabolic control and quality of life, to reduce diabetes-related complications, and to minimize healthcare costs. The community based program is an interactive, collaborative process that can equip the young adults as well as the adults with basic knowledge to manage their diabetes while focusing on their self-identified problems and goals. Thanks to our committed donors, funders, and partners, we provide a wide range of services on site, including podiatry services, diabetic shoes, preventive health screenings, vaccinations, patient education, select diagnostic testing, and laboratory services. Assyrian Family Health Alliance provides patients a true medical home, with ongoing, high-quality, compassionate care and the opportunity to build a relationship with their healthcare providers. Furthermore, at Assyrian Family Health Alliance we are committed to treating the "whole" person and are dedicated to finding new approaches to meeting the complex health needs of our patient population. Through an award-winning health education program and a growing social services department, we empower patients to take ownership of their health and well-being. Our Skokie health center is open for patient care 50 hours per week - offering appointments six days a week. All podiatry examinations, lab tests, medications, and services are provided at little to no cost to our patients.
