Mission: We are called to strengthen communities by empowering people to respond to life's challenges.
Programs: In home services - a new innovative alternative for older adults and persons with disabilities or chronic illnesses. We help these individuals to remain in their homes and communities to lead independent and satisfying lives by enabling them to select their care givers and manage their own care. Our self-directed care gives people choice and control over decisions about their personal needs. The consumer with assistance from ascentria care alliance in-home care representatives, if desired, hires and trains staff on their care needs. The consumer supervises the employee as well on a schedule that is mutually agreeable. This service is provided through medicaid or at a very competitive private rate.
