Mission: Alternatives promotes the independence and quality of life for older adults, adults with disabilities, and their families.
Programs: Care coordination program - provides ongoing assessment and service coordination for older adults to remain independent. In fy18, alternatives received 16,376 referrals to complete more than 12,546 in-home assessments. Alternatives maintained an ongoing caseload of 2,750 participants. As a program, care coordination assisted 95% of seniors eligible for nursing home care to remain in their home. 92% of participants and caregivers indicate they would recommend alternatives' services to others.
adult protective services - conducts intake, investigation, casework and intervention to prevent further abuse, neglect or financial exploitation of older adults or of adults with disabilities (age 18-59). In fy18, adult protective services investigated 810 cases this past year of which 30% of the cases were substantiated for abuse, neglect or exploitation. As a program, protective services staff were able to assist 86% of victims to reduce or eliminate risk. Caseworkers assisted 88% of victims to accept services to remian independent and free from further victimization.
ombudsman - provides advocacy, consultation and follow up for more than 10,000 nursing home residents in a 16 county service area. Ombudsmen provided 1,849 consultations in fy18 and assisted 326 nursing home residents to resolve complaints about their care. 95% of the quality measures for case interventions were met in fy18.
