Mission: Health care delivery & management: provide community based programs, services, and facilities to seniors, disabled and ill persons in their homes and communities. Collaborative care is provided by a team that includes nursing, therapy, personal care, home health aide, medical social work, and other related services. Services were provided regardless of race, ethnicity, creed, or ability to pay.
Programs: Vna alliance is an independent, voluntary, nonprofit home health agency licensed in pennsylvania. All services are available without regard to race, religion, creed, color, age, ancestry, national origin, sex, sexual preference, lifestyle, socioeconomic status, veteran's status or handicap. The agency strives to operate in a christian manner in all interactions with consumers, staff, and the community. The provision of care to consumers recognizes and supports consumers' rights and the inherent worth and dignity of each individual. Care is provided in a holistic, comprehensive manner that helps consumers achieve their desired quality of life and functional outcomes. Service to the community is achieved through responsive consumer care, caregiver support and education. Working as an interdisciplinary team, staff respect and support each other. Continuous quality improvement is integral to all agency functions with emphasis on employee development, risk management, strategic planning and organizational performance. The agency seeks to provide care to all consumers in need of its services while recognizing its goal to maintain financial viability. Service delivery is based on the belief that quality health care can be delivered using efficient, effective and ethical business practices. Approximately 2,228 persons were served with an average daily census of 273 patients. A total of approximately 35,293 (including 102 non-billable) visits were completed by various disciplines. Caregiver assistance and support is another identified need vna alliance addresses by providing training and information regarding health care concerns for the geriatric and difficult-to-deal with individuals, such as those with dementia or alzheimer's disease. We also service the corporate community through health and wellness initiatives including speaking engagements and flu clinics for agency residents and employees. Vna alliance continues to provide a wealth of community services to the residents, organizations and various groups in mercer, crawford, and venango counties and additionally reach communities in erie county. We have gained recognition as an agency that is respected as a contributing member of the community, truly making a difference in the lives of those residing in our geographical boundaries. The focus of our community engagement is geared primarily towards educating and screening the retirement and low-income populations, as these groups seem to have the greatest need regarding health screening and health education. We work closely with individuals who provide direct hands-on care or oversee the health care of these populations as well as general potential consumers of health care services in our communities. Additionally, as part of the prevention efforts for the deterioration of existing health conditions, vna alliance provides telehealth monitoring free of charge to patients with chronic diseases like congestive heart failure. Telehealth monitoring is a free service to the community. Vnaa utilizes approximately 20 telehealth units a month with patients. Vna alliance community liaisons have provided the following services: blood pressure screenings for st. Paul's colony residents, flu clinics: provision of approximately 250 flu vaccines to st. Paul's homes heritage assisted living residents, st. Paul's homes greenville ridgewood residents, st. Paul's homes shenango valley ridgewood residents, st. Paul's homes cottage colony residents, wesbury retirement community assisted living and independent living residents, and staff. Liaisons utilize educational materials in meadville medical center and grove city medical centers on chf and copd. Cross training educational programs to professionals in crawford and mercer county at the link collaborative at area agencies on aging, as well as education on chf and heart health on social media. Community trainings include provision of approximately 70 hours of community education while serving over 4,000 individuals. Multiple engagements at various senior centers within the geographical territory served were completed. Vna alliance is recognized as a wealth of information for local skilled nursing facilities, assisted living facilities and personal care homes in the community, frequently advising and assisting them to meet the needs of their residents. Vna alliance employees supported the community by collecting school supplies for our local elementary school. In addition, over the winter, a mitten tree was also assembled for the local students. During the thanksgiving holiday season the agency was instrumental in gathering food supplies to donate to our local family services food pantry for distribution to families in need.
