Mission: To provide caring and comprehensive health and supporting services, linking people experiencing homelessness to individual/collective solutions and to be a leader in creating service delivery models and solutions to homelessness.
Programs: Primary care services - program provides care for acute and chronic conditions through site-based and outreach services, with hospital referral as necessary. Psychiatry is provided through paid staff, and other specialty clinics such as eye, dermatology, podiatry, gynecology, and muscular-skeletal are staffed by volunteer providers. A motel voucher program is available for respite care (homeless with medical diagnosis requiring shelter during recuperation). This program has a medical/social services treatment team that focuses on the needs of homeless individuals, children and their families, as well as homeless adolescents.
social & behavioral health services - program provides a continuum of behavioral health services including: outreach to streets, camps, and shelters, a drop-in center providing linkages and referrals, psycho-social assessments, individual and group counseling and therapy, case management services including comprehensive case management for clients who have severe mental illness and/or co-occurring disorders, social work, and individual and group recovery services.
dental - the dental clinic provides comprehensive routine, emergency and preventative dental services. Occasional special needs cases are referred to dentists in the community willing to donate their time and expertise.
shelter plus care - the program provides rental assistance for people with disabilities, including mental health disorders and addictions. Participants in this program who have income are required to pay 30% of their income toward rent each month. Harm reduction program - the harm reduction program is a street outreach-based program offering prevention services to people using substances and at-risk for hiv and other diseases; syringe exchange out of a mobile van; and peer education groups, acupuncture detox, and other prevention education services. Artstreet - artstreet is a community-based art studio, including both open studio times for art-making as a way to reconnect homeless people with their community, as well as closed studio psychotherapy groups for clients in the long-term case management program and for clients in ahch residential recovery programs.
