Programs: Fiscal agent management and attendant care - fee-for-service based programs funded through several waivers sponsored by the department of welfare for aged and disabled consumers - additional fee-for-service funding is offered by several county contracts. 834 individuals with disabilities received assistance with employer related functions, payroll and support services to hire assistance workers to provide services for them in their home.
home modification and nursing home transitions - adjunct services supported by county and state funding on both a fee-for-service basis and actual cost to service basis. 47 individuals received home modifications in order to remain in their home and 101 individuals with disabilities were moved from nursing facilities to their home and in their community.
core services - funded by grants from the office of vocational rehabilitation - services include independent living skills, peer counseling, self-advocacy and information and referral. 3,705 individuals served and an additional 1,312 individuals with disabilities received a coordination of supports in order to remain in their home and in their community.
