Causes: Centers to Support the Independence of Specific Populations, Human Services
Mission: To bridge the gap in the delivery of home and community-based services for the aging and persons with disabilities in order to promote independent living.
Programs: Service/supports coordination is a service that provides the consumer/participant with a service/supports coordinator, who will assist the consumer/participant, with obtaining and maintaining services that are available in the home and community setting. These service allow the consumer/participant to live independently in their own home and to avoid potential placement in a nursing facility. A service/supports coordinator meets with consumers in their own home and provides advocacy and support.
