Results: Four Seasons is an independent, non-profit provider of trusted care for patients facing serious illness. In 2019, we celebrated 40 years of service to our community.
Target demographics: individuals facing serious illness and end-of-life issues
Direct beneficiaries per year: over 3,000 palliative care and over 4,000 hospice patients
Geographic areas served: 11 counties of western North Carolina
Programs: care navigation, home care, palliative care, hospice care, and grief support for children and adults.
