Mission: The Alliance for Positive Health (formerly the AIDS Council of Northeastern New York) is a not-for-profit human service agency whose mission is to reduce the impact and incidence of HIV/AIDS and other serious medical and social conditions.
Programs: Supportive services assists people living with hiv/aids in managing their illness. Care management, assessment, referral and follow-up, connecting individuals and families to housing programs, substance abuse treatment readiness, mental health care coordination, end of life planning, support groups, transportation to essential appointments, rent and utilities assistance and emergency financial support, food shelves, nutrition education and meal planning and social / recreational programs. Services also include enrollment in hiv related health insurance and insurance through the nys health exchange. 885 hiv positive persons were served during the fiscal year.
care management was provided to 714 individuals who are medicaid eligible and have a chronic illness such as hiv, diabetes, cardio vascular disease, obesity, or asthma. Care managers work with clients to help them navigate complex systems including health care and address other issues such as substance abuse, mental health, homelessness, domestic violence and provide support and follow-up to achieve positive health outcomes and build skills to help clients manage their lives. Services include: coordinate access to medical care, follow-up to assure client follows treatment, referral to substance abuse treatment and other services, family services, budgeting assistance, applying for entitlements, home visits and transportation.
prevention services facilitate changing behaviors that put people at risk for hiv, sexually transmitted infections and hepatitis c and increases awareness of unsafe decisions and practices. Services reflect best practices and respond to the rapidly changing trends in hiv disease. Prevention services include: individualized risk-reduction counseling community-based hiv/sti testing, counseling, hepatitis c testing, skills-building workshops, peer programs, outreach in high-incidence neighborhoods, internet interventions, transitional planning and testing in prisons, hiv prevention for at-risk youth and other high impact prevention programs.
