Mission: As a federally qualified health center, west county health center's (wchc) mission is to provide comprehensive, quality and accessible health care services to the communities of western sonoma county. A cohesive team of health care providers, support staff and volunteers are dedicated to wellness, compassion, affordability and excellence, while providing non-judgmental and equal care to all members of this diverse community. For the twelve months ended 12. 31. 2016; 12,923 patients were treated at the clinics representing 69,743 unique visits.
Programs: West county health centers, inc. (wchc) provides comprehensive health services for western sonoma county california, a rural and geographically isolated area located 75 miles northwest of san francisco. Since 1974, our health centers have been the sole health care providers for the under-served and marginalized in our 625 square mile service area which includes mssa 207 and mssa 210. 2. The total population is 47,595. (census 2010). Mssa 207 was designated as a hpsa in 1987, the first area in the united states to be designated based on its high incidence of aids, homelessness and poverty. In 1997, the service area was designated as a medically underserved population for the low income population, and in 1998, the area was designated a dental hpsa. In 2004, the office of statewide health planning and development split the boundaries of mssa 207 and this change created mssa 210. 2 where our occidental and sebastopol health centers are now located. Wchc provides comprehensive primary care services at four separately licensed primary care facilities - occidental area health center, russian river health center, sebastopol community health center, and gravenstein community health center. A fifth site was developed in 2016 to meet the needs of the homeless population in the service area. On-site services include: medical, integrated behavioral health and dental services; substance use treatment, case management; health education; and limited laboratory services. Additional services offered under contract or by referral agreement include: specialty medical and dental care; full-scope radiology services; full-scope laboratory services; and contracted pharmacy services at local pharmacies.
