Mission: Founded in 1901, visiting nurse services in westchester (vnsw) is a community-based, not-for-profit, medicare certified home health care agency serving westchester, putnam, dutchess, rockland and the bronx residents of all ages, their families and caregivers. Vnsw is a local agency serving local residents. The agency's services meet the rigorous community health accreditation program national standards of excellence for home care. Vnsw was the first home care organization in the area to achieve this standing. This accreditation continues to be awarded to our organization on an on-going basis. Programs: Vnsw provides a full array of comprehensive home health care services and utilizes a case management approach to ensure that patients receive all services that are needed for them to reach their maximum level of independence. Our core services include general medical-surgical care for adult, pediatric and adolescent age groups; maternal - newborn care, pediatric and adolescent care; and care of the terminally ill. A home care nurse serves as the case manager and works with the physician and all professional disciplines involved to establish and coordinate the plan of care. In addition to our home health care services, vnsw also provides the following innovative specialty services: advanced wound/ostomy care, mental health home care, cardiac disease management, pain management/palliative care, telehealth monitoring, joint replacement rehabilitation, dysphagia rehabilitation, tobacco cessation and vnsw cares; a geriatric care management program. The 2017 services included the following home health care services: skilled nursing - 87,856 visits physical therapy - 46,738 visits occupational therapy - 7,171 visits speech therapy - 2,830 visits medical social services - 2,674 visits home health aide services - 32,031 visits
in 2006, an innovative community service program, called the telehealth program, was created to assist our agency to achieve re-hospitalization rates below the new york state average. It was established to provide an enhancement to clinical management, monitoring, care and support through communication technologies. Also known as remote patient monitoring, personal health and medical data is collected from the patient while at home and transmitted to the home care agency via a web portal. This type of service allows a provider to continue to track healthcare data for a patient when not in the home. This effective patient monitoring can help to maintain the patient's health in their home and community, without having to physically go to a physician or emergency room, by reminding he/she to take theor medications, monitoring weight gain or loss and proactively sending a nurse to the home before a medical emergency arises.
