Programs: Home health: under the guidance of its community-based board of directors, the vna of st. Luke's, a member of the fully integrated st. Luke's university health network, provides medically necessary services to individuals irrespective to their ability to pay. When patients are referred by physicians or discharged from the hospital or other healthcare facility, the vna staff provides care in their homes. Home health services to patients, ranging from infants to seniors include: skilled nursing, physical therapy, occupational therapy, speech therapy, medical social services and home health aides. Luke's university health network, provides medically necessary services to individuals irrespective to their ability to pay. Hospice provides patients and their families with compassionate end of life care in their homes or an acute care inpatient unit. The focus is on care, not cure, and the quality and value of life, not the length. In most cases, care is provided in the patient's own home. community health: under the guidance of its community-based board of directors, the vna of st. Luke's, a member of the fully integrated st. Luke's university health network, provides three different community health programs: nurse family partnership (nfp), parent advocate in the home (path), and visiting nurse advocate for the county (vnac). These programs are focused on at-risk families with young children. Nurse family partnership, one of the vna programs, is a unique, evidence-based community health program that focuses on low-income, first-time pregnant women. Registered nurses work with the mothers-to-be during pregnancy to promote and teach positive health and attachment behaviors between a mother and her baby.
