Mission: To provide elderly or disabled persons with housing facilities and services sepcially designed to meet their physical, social, psychological needs, and to promote their health, security, happiness, and usefulness in longer living.
Programs: To provide elderly or disabled persons with housing facilities and services specifically designed to meet their physical, social, psychological needs, and to promote their health, security, happiness, and usefulness in longer living. The organization provides case management and supportive services to individuals with persistent mental illness in their own homes in union county, new jersey. The program is made up of two (2) small single-family homes, housing three (3) individuals in each, or six (6) individuals overall. The staff at the housing facilities provide supportive counseling, assistance in daily living skills, medication and symptom management, socialization, transportation, advocacy and problem solving. The program also provides rec recreational activities and linkages to appropriate educational/vocational and treatment services in the community. The staff is available 24 hours per day/7 days per week for supportive and crisis management. The staff is also available on an on-call basis for the remaining hours of the day/week.
