Mission: Twelve oaks is dedicated to providing quality housing, health and support services, primarily for older adults of all faiths that enrich the physical, social and spiritual dimensions of their lives. Twelve oaks is also committed to acting as a resource for the communities in which we operate.
Programs: Assisted living for seniors - twelve oaks provides assistance to individuals who need help with activities of daily living (adl's) such as bathing, dressing, ambulating and medication monitoring. They do not need 24-hour medical supervision but may benefit from specialized activity programs or memory support.
