Mission: The carmella rose health foundation was created to positively impact the underserved in our community by providing personalized one on one navigation services, eliminating barriers and connecting clients to available healthcare and social service resources. History: carmella rose health foundation was founded in 2004 out of the desire to help patients and families navigate the healthcare system. As a nonprofit 501(c) (3) organization based in cleveland, ohio, we help people access quality medical care during their healthcare journey.
Programs: We are unique because we are not connected to any one hospital system or federally-funded clinic. We also provide assistance to underserved and disadvantaged clients at over 35 sites in the greater cleveland area. Presently we help around 1,200 clients per year, and that number continues to grow. Our staff of community health workers, or chw's help improve quality of care by guiding patients through complex medical systems. Our team will serve as advocates and a consistent point of connection for resources to help patients overcome barriers they may encounter. In addition, we address health disparities, such as access to care due insufficient health insurance coverage, illiteracy, lack of social support and poverty. Through our program, our staff is able to impact the way a person deals with illness by building trust and helping people work through the hurdles that can sidetrack them from getting care. Our program benefits the community by educating people about the importance of having a medical home for non-emergency visits, thus providing better health outcomes and a healthier community.
