Causes: Centers to Support the Independence of Specific Populations, Human Services
Mission: The Bridge's mission is to change lives, by offering help, hope and opportunity to the most vulnerable in our community through mental health and substance abuse treatment, housing, vocational training and job placement, healthcare, education and creative arts therapy.
Programs: Residential services: the bridge houses and provides critical support services to approximately 800 individuals living in our 19 residences and more than 350 scatter site apartments. Three new buildings are in development and will house 180 more people. Graduated levels of housing - from supervised residences to independent apartments - are provided, to ensure clients receive the appropriate level of care. Specialized housing is available for mentally ill older adults, young adults aging out of the juvenile mental health system, and chronically homeless individuals. New housing is in progress for homeless veterans and low income families.
special populations: the bridge offers the following specialized programs: 1) the bridge's outpatient chemical dependency clinic provides treatment and support to clients diagnosed with a substance use disorder as they work to maintain their sobriety and manage their health; 2) the bridge has three act (assertive community treatment) teams, which provide case management, treatment, and support to individuals with mental illness who have barriers to succeeding in traditional treatment programs; 3) engaging individuals with complex medical conditions, the bridge's health home program provides care coordination services to clients to ensure that everyone involved in their health care is working together to provide informed, high quality treatment; 4) a comprehensive day program, pros (personalized recovery oriented services) is designed to help individuals with severe mental illness meet their self-defined goals, while also promoting improved social skills and reducing incidences of relapse and hospitalization.
clinical services: the bridge offers individual, group and family psychotherapy, where clients can receive therapy services in a confidential and empathetic environment. Medication evaluation, administration and monitoring are also available to bridge clients and community residents.
vocational services: the bridge uses a rehabilitation and recovery model that promotes independent living and self-sufficiency. For clients who are ready to explore employment opportunities, the bridge offers a wide array of service to help individuals through each step of the employment process. These services include diagnostic vocational evaluations, individual and group vocational counseling, job training, on-site internships, job placement, and post-employment support services.
