The three year program, offered in collaboration with united way emergency assistance agencies, offered direct financial assistance combined with intensive case management services. The program has been recognized nationally for its success in preventing homelessness.
secured funding and contracted for the community homeless management information system (hmis). The data gathered through our hmis informs all planning, decision making, and helps create successful programs that move people out of homelessness and into stable housing.
operated the central access point, a centralized emergency shelter hotline offering access to shelter and services for homeless individual men and families seeking emergency shelter, and to homeless prevention for individuals and families. Lead the implementation of homeless to homes, the community's comprehensive plan to put an end to homelessness. The plan, adopted by the city in 2008, recommends that homeless individuals have access to appropriate shelter facilities and comprehensive services to facilitate their movement to permanent housing.
