Programs: Osf healthcare ("osf"), hospital sisters health system ("hshs") and hospital sisters services, inc. ("hssi"), entered into a healthcare services transition agreement dated as of august 18, 2015 ("transition agreement"), pursuant to which: hshs ceased provision of inpatient services at st. Mary's hospital, 111 spring street, streator, illinois ("hospital"), effective january 4, 2016; hshs sold to osf certain assets that can be utilized for ambulatory facilities to be operated following closing of the proposed transaction; hshs donated to osf the land and buildings used for healthcare operations in streator; and osf will maintain ambulatory and primary care clinic services in the streator community. As such, the organization no longer operates as a licensed hospital, and this form 990 filing will reflect the limited financial activities of the winding down entity, and will no longer detail the activities of a hospital organization.
