Mission: The mission of the st. Johns place family center, hdfc (st. Johns place) is helping families achieve and sustain self-sufficiency. The agency is funded by the new york city department of homeless services. Services are provided seven days a week. Demographics- in fiscal year 2017, the agency served 810 individuals, 325 adults 21 and over and 485 children and young adults under 21. Of the dependent children and young adults that were served, 46 (10%) were infants and toddlers (ages 0-2 years), 86 (17%) were pre-school children (ages 3-4 years), 313(65%) were school aged children (ages5-17)and 40 (8%) (ages18-20 years)were young adults. The majority (98%) of the clients were minorities-either african american 64%)or hispanic 32%. Program staffing include masters-level social work supervisors, bachelors-level caseworkers and family activity staff. Core services- included access to medical and dental services, advocacy, comprehensive childcare, counseling, emergency food, financial literacy
Programs: Program accomplishments- both adults and children benefitted from services provided by st. Johns place. The following summarizes accomplishments for the fiscal year. Access to medical services- 374 clients were provided with access to medical and dental services. Clients received health screens, dental check-ups and access to specialty services. Clients who required disability determinations were referred tothe social security administration. Referrals for neo natal care were also made to the bureau of maternity services for neo natal care. Financial literacy workshops- an agency partner conducted financial literacy workshops. Referrals to employment services- 270 referrals for employment services were made during the fiscal year- on the average 48% of adult clients are either working or in job training. Transportation assistance- was provided to 216 clients who were escorted to public offices, fair hearings and housing searches. During the fiscal year of 2017 sjpfc assisted 80 families move out of shelter and into permanent residence. We were able to utilize a multitude of methods such as housing vouchers linc, city feps, section 8, and eosd (42%), nycha (24%), family reunification (25%) and independent relocation (98%). Sjpfc hosted 25 workshops for 370 families during 2017. Our staff created and utilized partnering agencies to conduct workshops based on the needs of the families. Some workshop topics consisted of financial literacy, developing presentation skills, interviewing, healthy living, housing applications, self empowerment and safe sleep for infants. Some of the partnering agencies we utilized consisted of capital one, dhs, and camba. Sjpfc made 498 employment referrals. Our employment specialist was able to make 354 employment referrals, assist 261 families with their resumes, and 70 were hired or enrolled in training. Advocacy- caseworkers helped 321 clients obtain public assistance and arrange for fair hearings if the city has made a mistake in calculating client benefits. Childcare services- 46 children participated in the infant care program. The after-school program served 54 children. In addition to nutritious meals, these programs helped children to build vocabulary and improve socialization skills. Emergency food- st. Johns place handed out 284 emergency food packets during the fiscal year. Caseworkers provided clients with the locations of the pantries and dates and times of food distribution. Newborn mothers were referred to the women infant and children program to receive wic. These mothers received supplemental food supplies throughout their pregnancy and for their newborn children. Family activities- several family activities were provided throughout the year. These included:trips to museums- 15 children were taken to brooklin children's museum. 20 children attended the liberty science center in new jersey. 15children also attended aviator recreational center at floyd bennet field in brooklyn. 40 participants including parents and children attended the universoul circus at roy wilkins park in queens. 20 students attended bowling in manhattan, 20 students attended rollerskating at prospect park skating rink. Drama, dance and exercise were introduced to the after school students by the department of education. 54 students also particaparted in different art projects throuhout the year, such as making christmas trees, christmas postcards, valentines cards, blackhistory month collages was done etc. Holiday program- included christmas parties for all residents and their children. Thanks giving turkey package giveaway for all 97 familiy residents. And several toy distribution for all the children in the facility. Edible schoolyard- st. Johns place in collaboration with settlement housing fund, inc. Maintained a vegetable and flower garden (garden). The garden combines hands-on classroom learning with healthy eating for the residents of crown heights, brooklyn. We were able to partner with a nutrionist and city harvest organization to teach our children the benefits of eating healthy fruits and vegetables. Mental health counseling- we parterned with new york center for psychotherapy
st. John's place has programs which provide food for children.
