Mission: St. John's community services-tennessee's mission is to advance community support and opportunities to people living with disabilities. The organization provides opportunities to help children and adults with disabilities successfully become a part of the community of their choice through support provided by dedicated staff. Sometimes successes are measured in imperceptible ways - from an individual's first step into the community, to an employer's greater appreciation for the talents of people with disabilities, to staff members taking ownership to create a more effective process to support individuals, to successful advocacy of government for greater financial support and access. Our many voices unite under one mission and meet opportunity with action to create social change and enable people to live their dreams and contribute to their communities.
Programs: St. John's community services - tennessee (sjcs-tn) is a subsidiary corporation of st. John's community services. The parent corporation is separately incorporated in the district of columbia and governed by a volunteer board of trustees. Each of the subsidiaries, including sjcs-tn, has a local volunteer board of directors and is supported by local leadership, with administrative offices located in the state of tennessee where the subsidiary is incorporated. Sjcs-tn contracts to use the copyrighted name "st. John's community services and acquires various administrative support services from the parent corporation. As a condition of the contract, the parent corporation establishes and monitors service quality standards and the subsidiary accepts these service standards as best practice. The administrative support services and technical assistance that the parent corporation provides include: accounting, cash management, evaluation and strategic planning, communication and information dissemination, human resources, information technology, quality assurance, corporate compliance and program development. Each of these support services is individually tailored to meet the unique needs of this subsidiary corporation and its distinctive cultural as well as financial status. These services assist sjcs-tn to meet the requirements of the contracts, laws and regulations under which it operates. Sjcs-tn operates under its own operating policies and procedures and local leadership. Administrative support services are provided by the parent corporation, st. John's community services, to the subsidiary corporation, sjcs-tn, at a low fixed rate consistent with national standards. To further the agency's mission of advancing community support and opportunities for people living with disabilities, the parent corporation sets quality standards consistent with the mission that must be met by the sjcs-tn subsidiary in the provision of services. Those services must foster inclusion, self-direction, economic self-sufficiency, relationship development and choice. Meeting and maintaining these standards is a requirement for the continuation of the administrative support contract and use of the corporate name. The parent corporation also provides technical assistance and training to the sjcs-tn subsidiary corporation to facilitate the attainment of these established standards. People receiving services through the st. John's subsidiary must meet local funding sources and the subsidiary's unique eligibility requirements for each specific service. Types of community support and opportunities for people living with disabilities that sjcs-tn provided during the 2017 fiscal year included employment services, community participation and community living. Since sjcs-tn added the individuals and staff from another provider, community developmental services in martin tn, there has been continual growth. This addition was at the request of the state department of intellectual and developmental disabilities (didd). While cds operated some existing community participation, community living and employment services, there were also facility-based day services, as well as residential habilitation services. During the entire reporting period, sjcs-tn has moved to add more community-based services to the entire sjcs-tn operations. This continues to be an area where sjcs leads the field and as a result, sjcs-tn is providing considerable support to others in the state regarding how to facilitate this model as a tn state standard. The total number of people with disabilities supported by sjcs-tn in fy 2017was as follows:- employment services: fy17 33 adults participated in job development, coaching, placement and on-going support in jobs working alongside employees without disabilities. - community participation: fy17 94 adults participated in this service that provides volunteer and community exploration, skill development and volunteer opportunities. - facility-based day services fy17 0 adults participated in this facility based day program. - facility-based employment-fy17 0 adults participated in this workshop. - residential habilitation group homes- fy17 15 adults lived large residential group homes. - community living/supported living: fy17 80 adults were supported to live in small group settings (no more than three) where they could enjoy all of the benefits of their chosen community.
