Causes: Centers to Support the Independence of Specific Populations, Health, Health Support, Human Services, Public Health
Mission: Senior pharmassist promotes healthier living for durham seniors by helping them obtain and better manage needed medications and by providing health education, medicare insurance counseling, community referral, and advocacy.
Programs: Senior pharmassist helped 2,317 individuals in the fy ended june 30, 2017. We provided comprehensive services to 546 seniors 60 and older in durham with incomes below 200 of the fpl. They benefited from medication therapy management with our clinical pharmacists trained in geriatrics, medicare insurance counseling, and tailored community referral to support independence. We helped 106 individuals 60 and older apply for part d extra help or low-income subsidies for medications and another 374 seniors received our direct financial assistance to help pay for medicines. We provided face-to-face medicare insurance counseling to an additional 1,264 medicare beneficiaries. Also, 498 individuals - not eligible for the services above - received tailored referralsto other programs, primarily for medication assistance. We began work with the duke geriatric workforce enhancement program to see older adults with complex needs as part of a of a hrsa-funded interdisciplinary care team. It began late in the fiscal year and we helped 13 older adults six were new to senior pharmassist.
