Mission: To promote independent and meaningful living for older adults through direct services and programs in the home and community.
Programs: Working with geriatric-trained care managers, care management helps older people to live safely and comfortably at home. The care manager meets the client in the home and develops a step-by-step, comprehensive care plan. The care manager implements the care plan serving as the consumer's liaison and advocate with the selected community resources and agencies. Services may include personal care, light housekeeping, home-delivered meals, adult day care, home modifications, health insurance counseling, medical supply reimbursement, emergency response systems and respite care for family caregivers. The care manager remains in communication with the older person to continue to guide the care and respond to changing circumstances. In 2016/2017, scs served 1,606 older adults in its long term care management program.
scs operates four nationally accredited senior centers in delaware county which nurture the mind, body and spirit of active older adults. Located in the city of chester, darby township, ridley township and yeadon borough, the centers offer a welcoming place to socialize with old and new friends, engage in fitness activities, educational and art programs, delicious meals and fulfilling volunteer roles. Offering information and assistance, the centers also refer older adults to other resources that help ensure continued independent living. Transportation is available for people needing a ride to the center. Almost 6,800 individuals made approximately 83,000 visits to scs centers in 2016/2017. Over 26,000 noon time hot meals, deli sandwiches and "southern grill" dinners were served at the centers. An outreach evening meal program at a low-income elderly apartment facility served an additional 3,150 meals to building residents and eligible community residents. The senior centers also prepared and delivered 130,452 hot dinners and cold sandwich suppers to 426 frail, low-income, homebound elderly. Fiscal year 2017 represented the fourth year of aging at home - a community network, a demonstration project conducted in partnership with united way of greater philadelphia and southern new jersey. In fiscal year 2017, this program saw the expansion to the ridley area. Aging at home aims to enable older people to remain living in their own home with dignity and peace of mind. As of 2016/2017, 812 "members" were enrolled, receiving door-thru-door transportation, vetted home repair referrals and volunteer home chore services. United way has awarded a fourth year "bridge grant" while scs continues its efforts to establish long-term fiscal responsibility. Scs scope of services also includes:apprise health insurance counseling - over 10,000 seniors received objective guidance on health insurance options one-on-one and in groups. Cognitive stimulation program - 1,071 brain fitness activity events conducted at nursing homes, assisted living facilities and senior centers. Volunteers - 53,118 volunteer hours by 375 individuals dedicated to scs programs and services.
the caregiver support program (csp) provides support and guidance for family members caring for elderly loved ones. The program provides free, compassionate counseling helping caregivers find positive ways to work with the challenges of insuring elderly loved ones are well cared for at home. The program links family caregivers to benefits, community resources and services. Financial support for caregiving-related expenses is also provided. Last year, the csp served 181 families. Approximately $272,500 in caregiver reimbursements was expended, providing financial support for personnel care services, caregiving supplies, assistive devices and home modifications.
