provided van service to residents for medically related appointments & therapies, pharmacies, banks, grocery & retail stores, church & other special pleasure trips per resident requests. There were over 1,260 riders served during 2017 from saucony cross and two other low income, senior citizen housing facilities in the kutztown area.
the service coordinator served an average of 18 residents per month, making referrals for home & community based services. Assistance with obtaining needed benefits, including health & prescription insurances and reduced motor vehicle registrations was provided. Counseling, case management, conflict management and rent rebate applications were also provided. Resident assessments were also completed to determine the level of need for services.
