Results: - A total of 9000+ referrals (Family Planning, HIV testing, Immunization, Antenatal, Nutrition, other facility services) from the community to the facility were made and 2791+ clients have been able to receive a service making effective referral. Similarly, 291+ referrals from the facility to the community were made and completed making it 98%. - 2000+ girls and women served with family planning methods - A total of 708+ pregnant mothers have been mapped, referred and linked for appropriate services like ANC and elimination of Mother to Child Transmission (eMTCT) and have successfully received the service. - 900+ Fruit trees planted with 300 PHE Model Households, and trained in alternative livelihoods. - 560 information sessions on Family planned held with young mothers groups and model village households -300 Smoke free -energy cooking stoves, 300 kitchen gardens and 300 drying racks constructed. -300 Hand washing kits given out to schools and health facilities -100 young mothers trained in saloon hair making skills and supported with start-up kits - 6 PHE clubs were formed in 6 primary schools
