Mission: To provide the highest quality of adult day health services to the community at a reasonable cost.
Programs: During 2017, parker at the pavilion served 244 (includes medical, social & parker day club at home programs) seniors and their families from the surrounding communities. The seniors have chronic medical conditions which prohibit them from being able to perform activities of daily living independently and cause them to be dependent upon a primary caregiver to remain in the community. The most common condition is a dementia illness such as alzheimer's disease. The primary caregiver is usually a spouse or adult child. The program offers the seniors health monitoring by a registered nurse including support with medication and disease management, such as diabetes and hypertension. In addition, the seniors benefit from therapy, transportation, nutritious meals, and social activity programs. The caregivers receive respite, education, and support services to assist the caregivers in managing the disease process, financial impact, and emotional stress of caring for someone who is chronically ill. Parker provides service to all eligible participants regardless of ability to pay. The parker at home program now brings the organization's social, wellness, and food services to four independent senior housing buildings in the community.
