Mission: To uniquely bridge health and social supports to preserve independence and dignity.
Programs: Pace program - the organization provides comprehensive, community-based care to frail, older adults, who meet nursing home level of care criteria. Care is provided both at home and at the organization's day center, and is coordinated by an interdisciplinary team. All products and services normally covered under medicare and medicaid are provided.
