Our integrated primary medical, dental, and behavioral health services result in consistently outstanding patient experiences and clinical results. Our clinical care team of 65 fte (full time equivalents), including 21 fte licensed independent providers, deliver services that include: maternity and newborn care (prenatal, deliveries including caesarean sections, postpartum and neonatal care, and preventive diagnostics). Evidence based medical, dental, and behavioral treatment for all ages. Laboratory, medication assistance, immunizations, and well-child exams, patient case management, cancer and other disease screening, well-child services, eye & ear screening for children, adult immunizations, and family planning services. Outreach and education (promotoras de salud), eligibility/enrollment services, interpretation, opioid addictions medication assisted therapy (mat), case management and peer support services, diabetes and maternity care coordination. Our various health promoter programs have received national awards. Special programs in areas such as diabetes education and case management, weight management, perinatal, and others have become part of the fabric of one community health. Two of our physicians are designated civil surgeons and provide medical examinations for immigration applicants. We partner with providence health and services in providing an integrated, rural, primary care residency program. We partner and coordinate with outstanding local providers for; behavioral health care, physical therapy, hospital care, massage therapy, acupuncture, and many other specialty and sub-specialty services according to patient needs. We increasingly coordinate with community based social service organizations for needs such as housing, employment, food insecurity, etc. We are proud to continue our mission of advancing health and social justice for all members of our community.
