Mission: To meet the individual, physical, spiritual, and medical needs of seniors in the community through on-site multi-disciplinary assessment,education, and on-going treatment or referral. To equip seniors, their families and caregivers with the tools, resources, and partners needed to maintain vitality and purpose in life based on christian truth.
Programs: Primary medical care provided for frail or disabled persons (physically or cognitively) with services performed "on-site" to meet special care needs. 549 patients were served.
